For Volunteer/Participant: Code of Conduct

Please retain a copy of this signed Code of Conduct for your records. By participating in MusicAid, I agree to follow these standards of conduct.

1. Treat all individuals with respect, dignity, and kindness. Listen attentively to others, value diverse opinions, and promote inclusivity. Refrain from engaging in any form of bullying, harassment, or verbal/physical abuse.
2. Demonstrate punctuality and reliability by attending all scheduled volunteer activities promptly and notifying the appropriate individuals in case of absence or delay.
3. Follow instructions provided by volunteer coordinators and other authorized personnel.
4. Use resources and materials provided by the organization responsibly and for the intended purposes.
5. Adhere to safety guidelines and practices, taking precautions to prevent harm to myself and others.
6. Dress appropriately for volunteer activities, adhering to any specific dress codes provided.
7. Resolve conflicts or disagreements in a respectful and constructive manner. Seek guidance from volunteer coordinators or appropriate individuals to address any issues or concerns that arise during volunteer activities.

Violations of this Code of Conduct may result in temporary suspension or permanent dismissal from participating in MusicAid activities. By signing below, I acknowledge that I have read and understood this Code of Conduct. I agree to abide by these guidelines and contribute to a positive and respectful environment.

For Participant's Parent or Legal Guardian:

Parental consent for media and photography: I hereby grant MusicAid, its representatives, volunteers, and any other affiliated individuals the irrevocable right and permission to capture, reproduce, distribute, display, and publish any photographs, audio recordings, and video recordings featuring the above-named student. I fully release MusicAid, its representatives, volunteers, and any other affiliated individuals from any and all claims, liabilities, or demands arising out of or in connection with the capture, use, and dissemination of the aforementioned media materials. I acknowledge and agree that the media materials may be used indefinitely and may be accessed by the general public. I understand that once published or shared, there is no guarantee of control over the use of these materials by third parties.

Student behavior guidelines: The above-named student agrees to adhere to the established behavior guidelines, code of conduct, safety instructions and policies set forth by MusicAid.

Release of liability: As the undersigned parent and/or legal guardian, I acknowledge and accept all risks associated with the student's participation in any and all activities organized by MusicAid. I, the undersigned, hereby declare my intention to be legally bound, on behalf of myself, my heirs, executors, administrators and assigns, forever waive, release, and discharge MusicAid, its representatives, volunteers, and any other affiliated individuals from any and all liability, claims or demands arising from or in connection with participation in these activities, including but not limited to any damage, loss or injury to the student, the student's property, the parent or legal guardian's property.

Permission release: I, as the undersigned parent and/or legal guardian, grant permission for the above-named student to participate in the events organized by MusicAid. I hereby certify, to the best of my knowledge and belief, that the aforementioned minor is mentally and physically fit and capable of participating in these events. In the event of illness or accident, I grant permission for emergency treatment to be administered. Furthermore, I understand and agree that I will assume full responsibility for any such action, including the payment of the associated costs. I affirm that I am fully aware of the inherent risks involved in participating in the aforementioned events. I acknowledge that the above-named student's participation in MusicAid activities is voluntary. I warrant and represent that I am the parent and/or legal guardian of the student. I affirm that I am entering into this Release freely and without any undue influence or constraint. It is my intention that this Agreement shall constitute a comprehensive and unconditional release of all liability to the fullest extent permitted by law.
